1 copy of this form must be submitted with any planning
application which generates an affordable housing requirement under
the Harrogate District Local Plan's affordable housing planning
policies. It must be countersigned by the Council's Housing
Development Officer before the planning application is
submitted.

| 'Extra Care Housing Proposal pro forma + Precedent Extra Care Accommodation Unilateral Undertaking' (42kb) |
1 completed copy of this pro forma must be submitted with any planning application for 'extra care' or 'close care' housing. Unless box 3 of the pro forma is ticked, 1 completed copy of the 'Unilateral Undertaking' must also be submitted with any such planning application. |
