What is the active health scheme?

For some people, getting active can be a challenge and they need some extra support. This is where the Active Health Scheme can help. It is an accessible service for adults who would benefit from a structured physical activity programme in a safe, supervised environment. The scheme will support people in making lifestyle changes especially in those people who have a limiting lifelong illness. The scheme will promote long-term participation in regular physical activity through goal setting and developing confidence therefore improving the person's mental, social and physical well-being.

Who is it for?

The Active Health Scheme is intended for people who have:

  • living within the Harrogate district
  • aged 18 years or above
  • low to moderate risk health conditions
  • severe coronary heart disease or early post myocardial infarction/coronary artery bypass graft/angioplasty (cardiac rehab classes only)
  • mild to moderate mental health problems (for example, depression, anxiety or stress)
  • diabetes type one and two
  • post physiotherapy musculoskeletal conditions (for example, knee, hip, shoulder)

Who should I talk to about the scheme?

If you think you may be eligible, please contact us or talk to a member of your healthcare team.

The medical professional will send a referral letter to the active health team. You will then shortly receive a phone call or letter inviting you to make an initial appointment with an exercise referral officer.

Connecting to well-being

For many people it can be difficult to make new friends, or simply find the courage to get out of the house and find an activity to participate in. It is often difficult to find out what is available in your community. This is where the connecting to well-being scheme can help. We offer a range of activities, some are designed to get you physically active but others are more about socialising, and the importance this has on your mental health and well-being.
